SERVICES

AGREEMENT

This agreement made by and between the Board of County Commissioners of Putnam County, Ohio, Fawcett, Lammon & Recker Insurance/by Dennis Recker, Ottawa, Ohio, and Fortman insurance Services/by Catherine Fortman, Ottawa, Ohio.

Mr. Love moved the adoption of the following Resolution:

WHEREAS, The Board of County Commissioners currently has health insurance through a consortium formed by the County Commissioners Association of Ohio called CEBCO.

and

WHEREAS, The Board of County Commissioners received bids for health insurance consulting services until 4:00 p.m. on the 31^st day of December, 2009.

and

WHEREAS, Bids for said consulting services were opened the 5^th day of January, 2010, and the lone bid received was from Fortman Insurance Services and Fawcett, Lammon, Recker who quoted 1.25% commission.

now therefore, be it

RESOLVED, The Board of County Commissioners does hereby award the bid for health insurance consulting services to Fortman Insurance Services/Catherine Fortman and Fawcett, Lammon, Recker and Associates, United Insurance Service, Inc./ Dennis J. Recker.

and be it further

RESOLVED, Fortman Insurance Services/Catherine Fortman, Ottawa, and Fawcett, Lammon, Recker & Associates, United Insurance Service, Inc./Dennis Recker will serve as agents for the county health insurance program for FY 2010.

and be it further

RESOLVED, Said agents will be paid a commission which equals one and one-fourth percent (1¼%) of the total cost of the insurance premium for 2010.
